Many of you have asked recently about implant protected occlusion and how it relates to both your provisional restorations as well as your final restorations. I have referenced in my seminars the article in Practical Periodontics and Aesthetic Dentistry, Vol 7 No. 5. Basically, it must be remembered that natural teeth have both a compressive and lateral component of movement. The literature as reported up to 30 microns of compressive ( intrusive ) movement and from 60 to 120 microns of lateral movement, depending on the tooth. The anterior teeth obviously have the most. In order to adequately protect the implant(s), one must have the patient bite in full force occlusion. This will allow for any potential intrusive and lateral movement of the natural teeth and show the clinician where the true interferences may be. Follow-up occlusal appointments should be scheduled within 10 days of delivery to insure that no new interferences have occurred. Remind the patient of what they are to " look for " when it comes to high spots and that they should call for an appointment if they notice a high spot prior to a scheduled appointment.
